What to Expect

Village Medicine is currently located in the heart of Wallingford, Seattle. We have a serene and comfortable boutique clinic.

Intake forms

In order to secure your appointment with a Village Medicine provider, we require that completed intake forms be returned through the patient portal. To register on the portal, please scroll up this page, and click on the “Patient Portal” button. Your intake forms ask for a comprehensive list of supplements and medications you are currently taking (you’ll need the information about brand, dose, etc.). The intake forms can take a good amount of time to complete depending on the amount of detail you’re willing to include about yourself. These forms not only help the providers synthesize your health history and goals but also demonstrates your commitment to your well-being by taking the time to complete them with care.

Your First Visit

Because we treat the whole person, your doctor or nurse practitioner takes time to get to know you. Your intake process includes:

 

  • A complete history of the presenting and historic conditions
  • Any pertinent physical exams
  • Ordering of any new labs
  • A personalized treatment plan
  • Bring a list of your questions
  • New patient first appointments are 60-90 minutes long, depending on your provider.
Follow-Up Visit for Established Patients

This visit includes:

  • A continued medical history
  • A re-evaluation of symptoms and clinical progress
  • A thorough reassessment and changes to your individualized treatment plan
  • A review of new lab results
  • Ordering of follow-up testing as needed
  • Each follow-up session varies depending on the unique needs of each patient and the complexity of the condition and treatment.
  • Feel free to discuss with our staff how long your follow up appointment should.

  • Adults: 30-60 minutes
  • Children: 30-60 minutes
Acute Illness or uncomplicated quick check-ins
  • Adults: 15-30 minutes
  • Children: 15-30 minutes
Telemedicine Appointments (video conferencing or phone)
