We are asking you to join us in a partnership of health-care change-making.

Insurance, health care costs, and investment into your healing process can often feel overwhelming. Being informed and understanding your specific coverage before engaging in our services ensures that we are all on the same page and can focus on what we all really want to spend our time and energy on: YOU and your health.

Our Commitment to you

Naturopathic and Integrative medicine are powerfully effective and safe forms of health care which are highly individualized for the patient being treated. With origins traceable to ancient history, naturopathic medicine combines the most effective Old World herbal treatments with the most modern scientific discoveries and health care techniques.

As one of our patients, you can expect to be treated in a holistic manner, with all aspects of the body and mind being considered in totality, instead of merely having your symptoms managed. We understand the body’s powerful ability for self-healing and recovery, and we go to great pains not to interfere with that ability. Instead we make a point of removing all obstacles, so that these processes can occur naturally rather than artificially.

Finally, we understand that good health is essential to everything you do, now and in the future, and that all your hopes and dreams will be founded on your good health. That is why we will stand alongside you, serving as your advocate and ally. We encourage a lifestyle that promotes healthy living to prevent future disease, and empowers you to become an active participant and director of your life.

What to Expect

Village Medicine is currently located in the heart of Wallingford, Seattle. We have a serene and comfortable boutique clinic.

Intake forms

In order to secure your appointment with a Village Medicine provider, we require that completed intake forms be returned through the patient portal. To register on the portal, please scroll up this page, and click on the “Patient Portal” button. Your intake forms ask for a comprehensive list of supplements and medications you are currently taking (you’ll need the information about brand, dose, etc.). The intake forms can take a good amount of time to complete depending on the amount of detail you’re willing to include about yourself. These forms not only help the providers synthesize your health history and goals but also demonstrates your commitment to your well-being by taking the time to complete them with care.

Please feel free to contact us with any questions about the forms
